Birth Certificate
2×2 Picture
Latest Report Card (if you have; can follow)

2. Then we will email you the Enrollment Contract, print, sign and email back to us.

OR Through Our Office:

1. Set an appointment by contacting 0915-5493883 two (2) days before your preferred day

2. Bring the required documents to our office:
